A small practical quiz

Dear Friends,
Today we will perform a small quiz.I want you to observe the Uploaded Video very carefully and answer the questions at the end of the post.
Now this an adult male patient aged 31 years and complaining of nasal blockage both sides since the last six months following an attack of severe cold.There is no h/o any allergy or Bronchial Asthma.There is no h/o epistaxis.
Now the questions...
1) What is apparently the pathology?
2) Which side is it arising from?
3) What is the cause of bilateral nasal blockage?
4) What is the Differential Diagnosis?
5) What is the Management of this patient?

  1. 1]the apparent pathology appears to be antrochoanal polyp arising as a consequence to an attack of cold leading to an infection

    2]polyp seems to be arising frm right maxillary sinus extending in d right nasal cavity

    3]the cause of bilateral nasal obstrusction is the growth of the polyp which has extended into the nasopharynx thus leading to blockage of the opposite choana

    4]d/d- large choanal polyp
    adenoid hyperplasia
    large benigh and malignant tumours

    5]management
    polppectomy
    in cases which have recurrance- caldwell-luc operation may be required to remove the polyp completely
    and endoscopic sinus surgeries are prefferred these days viz fess

  2. sir i have a question
    in cases of antrochoanal polyp does not recur after complete removal and ethmoidal polyp do is it because d latter is a single structure arising frm d sinus and d ethmoidal being multiple or any other reason is there????
